Obama hears job concerns on Google+ Hangout

Sunday night, Google+'s senior vice president of engineering Vic Gundotra?posted?to the social network that his team was "nervous" about its Hangout with the president of the United States. He said there were some technical challenges?? for instance: "You just don't walk into the West Wing of the?+The White House?and set up computer equipment."

Besides some slight technical difficulties at the beginning, the livestream Monday was nearly flawless on YouTube, and the first Google+ Hangout broadcast publicly with President Barack Obama lasted just over 49 minutes.

The start of the live broadcast was delayed 2-1/2 minutes.?Google+'s Steve Grove, who moderated the discussion, said at one point, "Can you hear me? You can't hear me, guys, you need the speaker on." Obama's voice was then heard: "Now I can."

The rest of the Hangout went fairly smoothly, with only an occasional lag, interruption or moment of confusion.

Viewers of the Hangout saw seven boxes at the bottom of the computer screen. One belonged to Grove. Five more to Americans who had been invited to participate, including a group of students from John F. Kennedy High School in Fremont, California. In the final box: the president.

All boxes were displayed equally, and whoever was talking lit up the top of the screen. It was a moment Googlers could likely only dream of when they conceptualized and developed Hangout, a group video chat generally limited to 10 participants.

Perhaps the most notable exchange during the Hangout was when participant Jennifer Weddel of Texas, whose husband has had trouble finding permanent employment for the past three years, interrupted the president.

Obama had said he had a list of folks looking to hire engineers, her husband's specialty. "There's a huge demand for engineers around the country right now," he added, when Weddel jumped in.

"Um, I understand that, but how ? given the list that you're getting, I mean we're not getting that ? You said in the State of the Union address that business leaders should ask themselves what can they do to bring jobs to America," she said.

The president had a very personal response which Weddel seemed satisfied with.

"If you send me your husband's resume, I'd be interested to find out what's happening," Obama said. "But the word we're getting is somebody in that high tech field should be able to find something right away."

"I'll have to take you up on that," she replied.

Another personal moment took place at the end, when Christine from Evanston, Illinois, asked a quick follow-up after her last question.

"Yeah, let me see them," he replied. And she brought her two sons and daughter into the viewing area.

"Make sure to work hard in school and do what your mom tells you," he quipped.

Tough questions
Not all questions were easy to answer.

A young student asked Obama what he would say to students afraid to go to college because they're worried about paying off student loans.

"I think young people have a little more responsibility to think ahead and make sure when you make that investment it's actually in pursuing a career in which you can have some confidence you're going to find a job down the road."

He also said he and first lady Michelle Obama took out loans themselves which are paid off.

When Weddel asked what the president would say to young Americans who have seen their own parents get laid off, and worry about debt and potentially facing the same situation, he said, "When times are tough, obviously a lot more people are concerned about taking on debt." But he added, "The unemployment rate for folks who only have a high school diploma is multiple times higher than for folks who have a college degree.

"As tough as this has been," he said, "the odds (are) that you'll do much much better in your lifetime, over the course of your career ... if you have a higher-education degree."

To a woman who said she has been unemployed for five years, Obama responded: "The biggest thing I can do for people out of work right now is to grow the economy."

A mother asked how parents should explain the economy to their children.

"We're going to be fine if we make some good decisions," he said, adding as president he'd like to instill hope in the American people.

He referred to manufacturing, which he discussed at length in his recent State of the Union Address.

"I want us to be a country that is building and selling products all around the world," he said, adding he wants to provide incentives for companies to invest here and insource rather than outsource.

It was such a pleasure and honor to be a part of it. The Google team who organized it was really mint, the President is fun and engaging. It went by faster than I ever thought.... :)

Ray tells NBC News he asked a question like everyone else and the next thing he knew he heard from Google and was told his question was picked to be put before the president, thereby allowing him to participant in the live-streaming event.

Grove said 135,000 questions were submitted ahead of the event, according to the New York Times' Liz Heron.

Former Italian president Scalfaro dies at 93 (AP)

MILAN, Italy ? Oscar Luigi Scalfaro, a past president of Italy who held the post during the sweeping corruption scandal of the early 1990s that reshaped the country's post-war political landscape, died Sunday in Rome. He was 93.

Italian President Giorgio Napolitano paid tribute to his predecessor as "a protagonist in the democratic political life" and an example of "moral integrity."

"As president of the republic, he firmly and steadfastly confronted one of the most difficult periods of our history," Napolitano said in a statement.

Pope Benedict XVI remembered Scalfaro as "a distinguished" Catholic man of state, who "helped to promote the common good and the perennial ethical and religious values."

Scalfaro was a key figure in postwar Italian politics, helping to write the constitution and to found the former Christian Democrats. He held numerous prominent government posts before becoming Italy's ninth post-war president, a position that is largely ceremonial but carries the significant role of moral compass for the country.

As president from 1992-1999, Scalfaro was often called upon to resolve Italy's recurrent political crises, either choosing a new premier or calling early elections. He once called Italy's volatile political situation "pathological."

The "Clean Hands" investigations launched in the early 1990s uncovered a broad system of bribes that wiped out much of Italy's political class, including key members of the conservative Christian Democrats and the center-left Social Democrats. The scandals deeply eroded Italians' trust in politicians and led to the demise of the two parties that had formed the pillars of post-war Italian politics.

Premier Mario Monti said Scalfaro "consistently defended the values" enshrined in the constitution "bearing witness with his actions and his rigor to all Italians, in particular the young."

A devout Roman Catholic with a law degree from the Catholic University of Milan, Scalfaro spent the World War II years working to help imprisoned anti-Fascists and their families.

Then, in 1946, he won a seat in the assembly that wrote the constitution for the Italian Republic, declared in late 1947 after a popular referendum abolished the monarchy.

Scalfaro, a native of the northern city of Novara, was elected to the Chamber of Deputies in the Italian republic's first general election in 1948 and remained a deputy until he was elected president in 1992.

Scalfaro held junior posts at various ministries through the 1950s and early 1960s. In 1966, he gained his first Cabinet position when Premier Aldo Moro appointed him transportation minister.

In subsequent governments, Scalfaro served two more stints as transport minister and was education minister and interior minister. He was vice president of the Chamber of Deputies from 1976 to 1983.

He became a senator for life after completing his term as president.

He is survived by a daughter, Marianna. A funeral is set for Monday afternoon in Rome.

Cuba Communist Party eyes term limits at gathering (AP)

HAVANA ? Cuba's Communist Party closed the first day of its national conference Saturday with declarations of support for the burgeoning private-sector class, tough talk on corruption and vows to be attentive to islanders' complaints.

But there were no concrete resolutions on President Raul Castro's call for political term limits or word on leadership changes during the closed-door gathering, a two-day follow-up to last April's historic party summit where delegates opened up long-shut doors of economic opportunity by green-lighting fledgling reforms.

While the government has essentially followed through on its economic promises like liberalizing home and car sales, expanding private-sector activity and offering loans to support farmers, entrepreneurs and homeowners, expectations were low that this weekend would yield any blockbuster news beyond what officials already hinted at.

"The expectations were high because this conference was perceived as an act of continuity with relation to the Sixth Congress, as a space to complete the economic adjustment with complementary political reforms," said Cuban-born economist Arturo Lopez-Levy, a lecturer at the University of Denver. "It became clear that that vision was unfoundedly optimistic."

Foreign journalists were not allowed access, and no major announcements were made through the island's official media.

State-run website Cubadebate showed photos of President Raul Castro presiding over the conference wearing a gray blazer and a dark, open-collar shirt, with what appeared to be a small bandage on the tip of his nose. There was no word of any appearance by Fidel Castro, who was greeted with a standing ovation and some tears at the April congress.

In a brief snippet of video posted on Cubadebate, Vice President Jose Ramon Machado Ventura said in a keynote speech that the conference would focus on "the everyday work of the organization."

Raul Castro himself dampened expectations for any big announcements, saying two weeks ago that nobody should have "great illusions about the conference" and the topics of debate were more "an internal question for the party."

According to a draft agenda that circulated last fall, one matter up for discussion will be his call to limit officeholders including the president to two five-year terms, with the goal of achieving a "gradual renewal in leadership."

Over the years a number of rising young stars considered potential successors to Raul Castro, 80, and his brother Fidel, retired and largely out of sight at 85, have been unceremoniously fired. There are no obvious replacements waiting in the wings.

In April the younger Castro, who is also party chief, lamented the lack of politicians prepared to take over from the 70- and 80-somethings who occupy many top posts. Grooming new leaders would be a priority during his five-year term, he said.

Recently a Cuban official told The Associated Press that despite the lack of movement in visible roles like cabinet ministers, many midlevel government posts have quietly changed hands, with younger officials moving up. If true, that would bolster Raul Castro's claim that his government was laying the groundwork, albeit slowly, for generational change. The official spoke on condition of anonymity because he was not authorized to discuss the matter publicly, and his assertions could not be independently confirmed.

Communist Party newspaper Granma said Saturday that delegates would consider how best to promote women, blacks and young people through the ranks of the party and government.

They also evaluated the party's role in "the direction and systematic control of the process of updating the economic model and the progress of the economy," Granma said.

The draft agenda for the conference was discussed at thousands of local party and committee meetings and many of its objectives modified. The purpose of the conference was to turn the agenda into policy in support of last year's reforms, Machado Ventura said.

"Let us make our best effort to cast off obsolete mindsets of inertia and old prejudices," he said in comments broadcast on the news. "Let us work with responsibility and an eye toward the future, with the goal of strengthening the foundation of the (party's) work."

The Communist Party, the only party allowed in Cuba, does not have lawmaking powers but issues guidelines that are later taken up by parliament.

The conference's opening day was timed to coincide with the 159th anniversary of the birth of poet and independence hero Jose Marti, in whose honor a noontime 21-cannon salute thundered from a historic fort overlooking the Havana harbor.

But some said that's about all the fireworks they expect this weekend.

"The only thing that might be interesting is if they have some kind of election, to see who they put in the political bureau, especially if they put someone there who's not 80 years old," said Javier Blanco, a 32-year-old Havana resident. "Other than that... I don't think anything will come out that we don't already know."

Many Cubans were disappointed last month when authorities scuttled a proposal to eliminate the exit visa required for travel off the island.

Lopez-Levy said recent signals from Cuban leadership suggest there's no prospect for major political change in the near future.

"The Cuban Communist Party is going to make an adjustment to its internal life ... but it's not going to subject its political model to open debate on structural reforms as it did with its economic and social project," he said. "It's a low-risk strategy for the short term, but it could complicate the future."

Some G20 countries soften stance on Europe: sources (Reuters)

MILAN/MEXICO CITY (Reuters) ? Some of the world's biggest economies want to move quickly on a cash injection for the International Monetary Fund to help rescue the euro zone, but hardliners may still scupper an early deal to boost the fund's war chest, G20 sources said on Friday.

Officials from the Group of 20 leading economies are engaged in what one called a 'chicken and egg' game as they work toward a possible deal on boosting the IMF's firepower at a meeting of the bloc's finance ministers and central bank governors in Mexico City in one month's time.

Emerging market powers Brazil and China are among the countries keen to pursue the two-track plan pushed by the current G20 president Mexico to work on additional IMF funding simultaneously with extra steps from Europe, one G20 official told Reuters, rather than insisting on European action upfront.

"There was a much more cooperative sentiment between G20 countries than in recent meetings," said the official, referring last week's discussions between G20 deputies in Mexico City.

"Some emerging countries are more open to consider contributions to increase IMF resources in parallel with euro zone efforts, so they are open to make commitments to increase IMF resources in the next few weeks," the source added.

Mexican central bank governor Agustin Carstens said a consensus was building on boosting IMF resources to help European countries and others that need aid.

But the February 25-26 meeting deadline may prove ambitious, given the United States' insistence that Europe boost its own crisis shield further before any pledges to the IMF - which estimates it needs $600 billion more to limit the fallout.

"Our view is that the only way Europe is going to be successful in holding this together is for them to bring a stronger firewall," U.S. Treasury Secretary Timothy Geithner said at the World Economic Forum in Davos, Switzerland.

"If Europe is able and willing to do that, we believe the IMF is ready to play a constructive role."

Canada is also taking a tough public stance, although a G20 official from another country said Ottawa was becoming more conciliatory, along with Japan.

"Canada and Japan are more flexible than in the past," the second official said. "It could be a bit more difficult with the USA, although they too have softened their position, but it's still early in the game."

STANDOFF FEARS

Europe, for its part, supports the two-track approach, but officials are concerned that Germany's reluctance so far to back increased funding for the euro zone's own rescue fund may fuel a standoff at the G20.

Germany has insisted that the safety net should not exceed 500 billion euros, but officials close to the G20 talks estimate that a further 230 billion to 250 billion euros is needed.

"It is important that we should not let this be locked between the Americans and the Germans, or the IMF and the Germans, so that nobody would get any pretext or excuse to not do their part," one senior euro zone official said.

A G20 official from a large emerging market economy said Europe accepted the need to put in more resources but "won't say it for fear of" Germany.

"They will get to that point because they know not one cent of this IMF money will be made available unless they come up with their side ... the majority view is that we move in parallel we have things ready, but we don't have to deploy it until the Europeans have gotten their act together."

European Union leaders will discuss increasing the bloc's permanent rescue fund, the European Stability Mechanism (ESM), on March 1-2, just days after the February meeting in Mexico, with the timing creating extra difficulty for policymakers.

"If the parallel approach wins inside the G20, a deal on increasing IMF resources could be clinched by the G20 meeting in February," the initial G20 source said. "Otherwise, the G20 will work on reaching a deal by next April in Washington, after an increase of ESM firepower is signed in March among euro zone countries."

G20 finance ministers are due to meet in Washington on April 19-20 ahead of a leaders summit in Mexico's Los Cabos on June 18-19.

Countries keen on the parallel approach are Brazil, Australia, Japan, Indonesia, China, Indonesia and South Korea, the source said.

A senior Brazilian government official confirmed Brazil was keen to push the two aims simultaneously, but said a commitment to a bigger ESM would definitely smooth the way.

"If the Europeans increase (funding to) the ESM then they increase the chances of additional resources to the IMF in support," he said.

The extra funding may come in the form of bilateral loans between individual countries and the IMF or an increase in countries' quotas, which could also give emerging economies more say in how the fund is managed.

APNewsBreak: Obama to protect US goods globally (AP)

DAVOS, Switzerland ? President Barack Obama has adopted a new strategy declaring for the first time that the United States has a national security interest to protect the nation's economic goods against terrorists, criminals and natural disasters in all corners of the globe.

The new U.S. policy unveiled Wednesday by Homeland Security Secretary Janet Napolitano in Switzerland is called the "National Strategy for Global Supply Chain Security," according to a White House document provided to The Associated Press.

It says potential economic threats to goods dependent on supplies from beyond U.S. borders are now a matter of national security and that the government must "resolve threats early."

And that's not just cargo shipments ? all "cyber and energy networks" also are affected.

At the World Economic Forum in Davos, she called Obama's policy "a look across all of U.S. government" preparing for the worst. "When the unknown occurs, you have no time," she said.

Businesses have often sought to cut costs by farming out many parts of their operations, leaving them potentially more at risk to disruptions outside the confines of their traditional areas of management. But with the economy increasingly globalized, businesses are becoming more dependent on each other ? and more exposed to these risks.

"The global supply chain provides the food, medicine, energy and products that support our way of life," the document says.

"Many different entities are responsible for or reliant upon the functioning of the global supply chain, including regulators, law enforcement, public-sector buyers, private-sector business, and other foreign and domestic partners," it says. "The system relies upon an interconnected web of transportation infrastructure and pathways, information technology, and cyber and energy networks."

Daniel J. Brutto, president of UPS International, said more such cooperation internationally would not only address supply chain threats but also speed up trade. "Many countries want a paper chain system, an archaic system, that slows down commerce," he said. "When there are disruptions, you are penalized very strongly and I don't think the financial institutions take that into account."

The Obama administration cites as reasons for the policy the 9/11 attacks and more recent plots involving air cargo shipments filled with explosives shipped via Europe and the Middle East to the United States, according to White House documents. Other events that have led to the change, the documents say, include Hurricane Katrina in 2005, the Interstate 35 bridge collapse in Minneapolis in 2007, Iceland's volcanic eruption in 2010 and Japan's earthquake and tsunami last year.

MIT professor Yosef Sheffi, an engineer who is an expert on risk in supply chains, said many companies can't even identify all of their suppliers. He said one company that makes iPhones, for example, has about 400,000 suppliers.

Obama, in a preface to the new policy, which is effective immediately, that "the global supply chain system that supports this trade is essential to the United States' economy and security and is a critical global asset."

The policy follows in the wake of a series of major natural disasters whose effects spill beyond one nation's borders.

"We have seen that disruptions to supply chains caused by natural disasters ? earthquakes, tsunamis and volcanic eruptions ? and from criminal and terrorist networks seeking to exploit the system or use it as a means of attack can adversely impact global economic growth and productivity," the president wrote.

"As a nation," he added, "we must address the challenges posed by these threats and strengthen our national and international policies accordingly."

The March tsunami in Japan, for example, was devastating for that nation's economy and temporarily disrupted the production of automobile makers and other manufacturers.

Car exports, too, declined after the recent flooding in Thailand, where many Japanese automakers have assembly lines. Iceland's volcanic eruption in 2010 paralyzed air traffic, affecting passengers and cargo around the world.

The White House "strategy" is not an executive order. But it instructs federal agencies to immediately focus on "those components of the worldwide network of transportation, postal and shipping pathways, assets, and infrastructures by which goods are moved until they reach an end consumer."

It also suggests that all U.S. trade partners should be pressed to agree to what Obama calls "information-sharing arrangements, streamlining government processes, and synchronizing standards and procedures."

The strategy has far-reaching implications. It not only would apply to all cargo goods entering the country by ship, airplane or truck ? the U.S. already inspects all of what it considers to be the highest-risk cargo ? but also could set the stage for U.S. action to strengthen the security provided in other countries.

Obama is requiring all federal agencies and departments to report back to him within a year on how their efforts are going and make "recommendations for future action developed during the outreach process" of talking with other countries.

PARIS (AP) ? The former head of a French company at the center of a breast implant scandal affecting tens of thousands of women worldwide was arrested along with his former deputy Thursday in southeast France, officials said.

Jean-Claude Mas, who founded and ran the now-defunct implant maker Poly Implant Prothese, was detained as part of a judicial investigation in the southeastern city of Marseille into manslaughter and involuntary injuries, an official said. A regional official said the company's former No. 2 executive, Claude Couty, was also detained.

The arrest before dawn at a family residence in the Mediterranean resort town of Six Fours Les Plages culminates weeks of speculation about whether judicial investigators would be able to assemble enough evidence to detain Mas on legal grounds.

Mas, 72, had been believed to be residing in the home. His defense lawyer Yves Haddad earlier this month denounced the "numerous un-truths, nonsense and aberrations" in the case, but said Mas would only speak with authorities.

So far no specific defendant has been named, but an official with knowledge of the probe said Mas was expected to face preliminary charges after appear before an investigating judge in Marseille later Thursday. He was being held in resort town ahead of that anticipated transfer.

The three officials all spoke on condition of anonymity because the case is in the hands of judicial investigators.

Investigating judge Annaick Le Goff opened the probe after a woman in the southwestern Gers region filed a lawsuit in the wake of the 2010 death from cancer of her daughter who had received a suspect implant.

Since then, a complaint by the sister of Marseille resident Edwige Ligoneches, a breast implant recipient who died in November of complications from lymphoma, has since been included in the case file ? along with as many as 3,000 other complaints by other alleged victims.

Le Goff also spoke with the head of a victims' association, Muriel Ajello, on Thursday, the regional official said.

A secretary at the Toulon office of Haddad told The Associated Press that the defense lawyer was with Mas during police questioning, and was not immediately available for comment.

The suspect PIP implants have been removed from the marketplace in several countries in and beyond Europe amid fears that they could rupture and leak silicone into the body.

Mas had run PIP until the company was closed in March 2010.

Mas is also on Interpol's most-wanted list, but the international police agency said its "red notice" was issued in June at the request of Costa Rica, where he faces a drunken driving charge.

Authorities worldwide have been scrambling to strike a proper public response to the scandal, notably concerning who will pay to remove the implants made with cheap, industrial-grade silicone instead of medical-grade gel ? or if the implants need to invariably come out.

European governments have taken different positions: German, Czech and French authorities say they should be removed, while Britain says there is not enough evidence of health risks to suggest removal in all cases.

On Wednesday, health authorities in Brazil said the government will fine private health plans that refuse to pay for the removal and replacement of faulty breast implants sold by PIP and a Dutch company.

The scandal has put pressure on French health authorities for allegedly not doing enough to vet the quality of a product used by untold thousands of women both in France and abroad.

France's Health Safety Agency has said the suspect implants ? just one type of implants made by PIP ? appear to be more rupture-prone than other types. Investigators say PIP sought to save money by using industrial silicone, whose potential health risks are not yet clear.

PIP's website said the company had exported to more than 60 countries and was one of the world's leading implant makers. The silicone-gel implants in question are not sold in the United States.

According to estimates by national authorities, over 42,000 women in Britain received the implants, more than 30,000 in France, 9,000 in Australia and 4,000 in Italy. Nearly 25,000 of the implants were sold in Brazil.

Device makers urge coverage of weight-loss surgery (Reuters)

CHICAGO (Reuters) ? Device manufacturers are pushing the government and health insurers to cover weight-loss surgery, an effort that could give millions more obese Americans access to the treatments.

Advocates say it will give obese patients a complete arsenal for fighting the condition that can spur a host of life-threatening illnesses and help save billions of dollars in healthcare costs for employers and the government.

Critics argue that bariatric surgery has high rates of complications and that, ultimately, surgery does not change the behavior underlying obesity.

One of the most vocal manufacturers is Allergan Inc, the maker of Botox and breast implants. The company wants to revive weak sales of its LapBand, a silicone cuff that is implanted around the top portion of the stomach to constrict food intake.

"It's all about reimbursements," Allergan Chief Executive David Pyott told Reuters. The benefits of weight loss surgery, he added, "are not well understood by policy makers."

Allergan recently beefed up its staff working on securing reimbursement for LapBand to more than 100 people from seven.

Pyott is spending more time in Washington D.C. speaking with officials at the U.S. Department of Health and Human Services, as well as with lawmakers about having bariatric surgery included in the package of standard benefits that all insurance plans must offer under the 2010 Affordable Care Act health system overhaul.

LapBand competes with a device called Realize made by Johnson & Johnson, which says it is also working toward better reimbursement.

Gastric banding is only one type of bariatric surgery. Others are more complicated, involving stapling portions of the stomach to limit food intake or re-routing the path of digestion, limiting calorie absorption. As with any major surgery, all carry the risk of complications and infection.

Allergan is pushing for coverage for all bariatric surgery, including methods that compete with its LapBand device, because it is more likely that private and public insurers would approve the entire category.

The company says some private insurers have changed their policies as a result. For example, the Midwest Blue Cross/Blue Shield Plan and the Health Alliance Medical Plan in Southern Illinois and Iowa recently eliminated the requirement that a patient must have tried and failed to lose weight under the supervision of a physician before getting surgery.

COSTS IN THE BILLIONS

Nearly 73 million Americans are considered obese, defined as having a ratio of weight to height, or Body Mass Index (BMI), of more than 30. For example, a 5'9" adult weighing more than 203 pounds is consider obese. About 12 million people are classified as morbidly obese, defined as having a BMI over 40.

The condition is the second leading cause of preventable death in the country behind smoking, as it can cause type 2 diabetes, heart disease, stroke, osteoarthritis, gall bladder and liver disease and many types of cancer. It leads to nearly $150 billion in annual U.S. healthcare costs.

A survey by Hewett Associates found that 45 percent of U.S. employers cite obesity as one of the most significant health concerns for their businesses, but many of their insurance plans - 44 percent of those with 5,000 or more employees - do not cover bariatric surgery.

Bariatric surgery coverage is often viewed as a separate benefit, said Gus Georgiadis, president of Triad USA, an employee benefits services and consulting firm. Larger employers tended to have better coverage.

Employers who do cover surgery often define patients as eligible at a higher BMI and require them to first try other methods for an extended period of time - even if they already have already made such attempts - and undergo a psychological evaluation. Most plans have high co-payments of around $5,000 to $7,500, making it too expensive for many.

"If you're making $25,000 a year and you have a $5,000 co-pay, that's 20 percent of your salary," said Joseph Nadglowski, President and CEO of the Obesity Action Coalition, a patient advocacy group. "High co-payments and the hoops and hurdles insurance companies put in front of surgery is limiting."

But Nadglowski still wants to see patients first try diet and exercise programs, followed by medications and then surgery as a last resort.

"There's a gap between Weight Watchers and surgery," Nadglowski said.

His organization is pushing for more access to all kinds of treatment, including nutrition counseling.

Georgiadis argues that certain treatments are more or less appropriate for a patient, given their condition.

"Diet and exercise at a BMI of 35 or greater will fail more often than it will succeed," he said.

Studies show that bariatric surgery is almost 10 times more effective for losing weight and keeping it off than other approaches. Some research has shown that gastric bypass is superior to banding, with bypass patients losing more weight and keeping it off better than patients who got the band.

Patients who choose gastric bands may do so because the surgery is less invasive and the device can be removed. But they require more maintenance and follow-up doctor visits for adjustments.

The band sometimes slips from where it was placed and in rare cases can erode into the stomach, complications that require removal.

The number of all types of bariatric surgeries was down about 8 percent in 2010, a year that saw a decline across the board in medical procedures as many Americans lost health insurance when they lost their jobs. High co-payments are also to blame.

Although just a small part of Allergan's revenue, LapBand sales fell 14 percent from a year ago to $156 million in the first 9 months of 2011, even after the Food and Drug Administration lowered the weight requirement to get the device.

STATE BY STATE COVERAGE

Some states already require some level of coverage.

Virginia and Georgia, for instance, mandate that state-regulated insurers offer bariatric surgery, but employers are not required to buy that coverage. Michigan has a unique provision that says all "medically necessary" procedures, not just bariatric surgery, are covered.

The government's Medicare and Medicaid health programs for the elderly and the poor, respectively, cover bariatric surgery and use the same guidelines as private insurance companies.

Convincing employers has been challenging, especially if they have a high turnover of employees, said Dr. Richard Feifer, Medical Director for National Accounts for Aetna Inc, the third largest U.S. health insurer.

"Employers who have significant turnover every year may not want to invest in bariatric surgery for employees who may not be working for them in 2 or 3 or 4 years when the benefits start to accrue," he added.

House votes on permanent electronic duck stamp (AP)

WASHINGTON ? The e-Duck Stamp would become a permanent part of federal law under legislation passed by the House Monday.

Since 1934, migratory waterfowl hunters over the age of 16 have been required to buy the federal migratory bird hunting and conservation stamps, or duck stamps, at post offices or sporting goods stores.

But four years ago, eight states joined a pilot program allowing them to sell temporary duck stamps through the Internet. The bill's sponsor, Rep. Rob Wittman, R-Va., said that program has been a success and it was time to make it federal law.

The bill passed 373-1. If passed by the Senate, it would give the U.S. Fish and Wildlife Service authority to make that program permanent and extend it to all states.

The electronic stamps are valid for 45 days, giving hunters and other purchasers time to get their actual stamps through the mail.

The program also makes it easier for hunters to go online to buy their federal stamps and state hunting licenses at the same time.

The stamps now cost $15 a year, with 98 percent of the revenues going to buy or lease wetland habitat for protection in the National Wildlife Refuge System.

The Migratory Bird Conservation Fund has received more than $800 million since 1934, putting 5.3 million acres of wetlands under the refuge system. The program currently generates about $25 million a year.

The stamps also give birders and other non-hunters free access to the nation's 553 wildlife refuges.

The U.S. Fish and Wildlife Service sponsors a contest every year where wildlife artists from around the country vie to have their entry selected as the design for the stamp the next year.

Canadian man gets 18 years for credit fraud (AP)

FARGO, N.D. ? He was unemployed and receiving welfare, but Adekunle Adetiloye was somehow still living lavishly, complete with a Range Rover vehicle, extended trips to England and an expensive condominium.

That alone piqued authorities' interest, but then there were two credit cards tucked away in his wallet that seemed to confirm suspicions that the Canadian man was up to something nefarious. The pieces of plastic each bore different names ? Donald Douglas and Vincent Andriole ? and helped authorities prosecute a case they describe as one of the largest high-tech bank robberies in U.S. history.

Adetiloye was sentenced Monday to nearly 18 years in prison on fraud charges. During the hearing in North Dakota, Assistant U.S. Attorney Nick Chase said the 40-year-old had an "insatiable hunger for other people's money."

Authorities believe Adetiloye masterminded a scheme to open nearly 600 fraudulent bank accounts and bilk 22 major banks out of hundreds of thousands of dollars. U.S. District Judge Ralph Erickson handed down a 214-month prison term and scheduled a Feb. 15 hearing to discuss returning nearly $1.5 million in losses to credit card companies and banks. The judge has said losses may have been as much as $5 million.

"Characterizing this fraud scheme as massive, if anything, is an understatement," Chase said in court documents.

Defense attorney Richard Henderson had asked for a sentence of fewer than 16 years. Henderson said any prison time for his client, a native of Nigeria, is more difficult than it would be for American citizens because he has no family in the United States. No decision has been made about whether he plans to appeal, said Neil Fulton, lead federal public defender for North Dakota and South Dakota.

"The sentence imposed today should send a strong message to those who would seek to scam the citizens and businesses of North Dakota and the United States," U.S. Attorney Timothy Purdon said in a statement released Monday. "We take the growing problem of foreign financial fraud seriously here and seeking justice for the victims of such crimes is a priority for our office."

Attorneys from both sides declined to detail the case ahead of sentencing, but investigators' efforts to deconstruct the multifarious case are laid out in the nearly 12,000 pages of court documents filed by lawyers in federal court.

Greg Krier, lead credit card fraud investigator for U.S. Bank, testified that it was the most complex case he had ever seen. His company, which has its own fraud unit, launched special training sessions focusing on the case in hopes of catching the culprits.

The case wound up in North Dakota after U.S. Bank's customer service center in Fargo intercepted calls by Adetiloye and others. The complexity of the scheme, which took five years to investigate and litigate, was highlighted in a sentencing phase that has lasted nearly a year and included numerous hearings and briefings.

The lead investigator, one of 25 people who worked on the case, put in 2,000 hours, authorities said.

Defense attorneys had argued that their client, the only person charged in the case, was a "marginal and minimal participant" whose role was to handle mail and withdraw money from ATMs. The government and the judge have said otherwise.

Investigators said the operation accessed information of nearly 16,000 people, about 500 of whom had their identities stolen for the purpose of obtaining credit cards. It's alleged that more than 100 commercial mailboxes were opened under false or stolen identities.

The government said Adetiloye went so far as to mask his handwriting after a judge ordered a test of his calligraphy

Erickson, the federal judge, said in court documents ahead of the sentencing that the evidence "indisputably demonstrates" that Adetiloye was a leader or organizer of the scheme. The judge has calculated losses to banks at about $1.5 million, but said it could have been as high as $5 million if credit limits had been maxed out.

The trauma cannot be measured, Erickson said.

"The non-monetary harm to the victims was substantial," the judge wrote. "They lost sleep, they lost time with their families, they lost time at work, and they lost their sense of security. Some victims spent hours trying to reclaim their credit record and their identities."

Court documents show that U.S. Bank suffered the most number of tainted accounts, at 130, for a total loss of about $76,000. The companies alleged to have lost the most money were Citibank, at about $271,000, and Discover, at about $248,000.

Brett Bogan, the security investigations manager at Reed Elsevier, the parent company of LexisNexis and ChoicePoint, told the court that data breaches of this type are extremely rare and knew of only one other case like it. He said the company sent out notices to more than 32,000 people whose personal information was compromised by the scheme.

"With their combined extensive and nationwide perspective, those entities place this fraud scheme at or near the top of their historical lists in terms of size and complexity," Chase said in court documents.

Rates rise at weekly Treasury auction (AP)

WASHINGTON ? Interest rates on short-term Treasury bills rose in Monday's auction to the highest levels since August.

The Treasury Department auctioned $29 billion in three-month bills at a discount rate of 0.04 percent, up from 0.025 percent last week. Another $27 billion in six-month bills was auctioned at a discount rate of 0.07 percent, up from 0.06 percent last week.

The three-month rate was the highest since three-month bills averaged 0.045 percent on Aug. 8. The six-month rate was the highest since these bills averaged 0.08 percent on Aug. 16.

The discount rates reflect that the bills sell for less than face value. For a $10,000 bill, the three-month price was $9,998.99 while a six-month bill sold for $9,996.46. That would equal an annualized rate of 0.041 percent for the three-month bills and 0.071 percent for the six-month bills.

Separately, the Federal Reserve said Monday that the average yield for one-year Treasury bills, a popular index for making changes in adjustable rate mortgages, was unchanged at 0.11 percent last week, the same as the previous week.

LONDON ? A report by the global music industry lobbying group says the growth in digital revenues remains stuck in the single figures.

The International Federation of the Phonographic Industry blames piracy and government sluggishness for the failure of online business to take off.

While a report out Monday says that digital revenue has risen by 8 percent over the past year one analyst says that isn't nearly enough to make up for the decline in sales elsewhere.

Independent media analyst Mark Mulligan says that in Britain and the United States "we've already lost half of the music market in the past 10 years."

IFPI chief Frances Moore acknowledged that digital growth "should be much higher" but said that widespread piracy still posed a challenge to the industry.

The US share of all international travel declined from 17 percent in 2000 to just more than 12 percent in 2010, the tourism industry says, costing the US nearly 80 million overseas arrivals, $37 billion in tax revenues, and more than 467,000 jobs.

The White House, claiming a modest increase in the US take of international travel could mean a million new jobs, insisted the president?s trip was part of an economic initiative and not political. But the trip fit a pattern of the president making campaign jaunts to states at about the same time they are choosing among the Republican presidential candidates.
